Aim

The award is in recognition of those individuals or teams that have demonstrated excellence in supporting patients who live with chronic cardiac illness. Nominees will have established relationships with patients and /or their caregivers and MDT. They will demonstrate evidence-based practice, knowledge, and promotion of well-being, with an emphasis on maximising resources and the persons quality of life.

Evaluation criteria

Application provides evidence of:

Promotes patient well-being.
Supports MDT and carers/families.
Demonstrates evidence -based practice.
Specialist area of expertise evident.
