Aim

This award is in recognition of the contribution to the implementation of SlainteCare programme in Ireland. The nominees should have made a significant contribution to the development and implementation of the programme through establishing pathways for integrated care and chronic illness management.

Evaluation criteria

Application provides evidence of:

Promotes early intervention and timely access to care.
Supports integrated community care.
Promotes primary /secondary prevention.
Shows innovation and resourcefulness.
