Aim

The award is centred on teams or individuals who have demonstrated excellence in primary, secondary and/or tertiary prevention associated with any form of cardiovascular illness. Nominees will have demonstrated a focus on the promotion of patient well-being and will optimise opportunities for health maintenance.

Evaluation Criteria

Application provides evidence of:

  • Promotes health and well-being.
  • Empathy and positive relationships with and for patients.
  • Evidence of knowledge attainment and dissemination.
  • Positively impacts patients’ lives and attitudes to health maintenance.
